Dr. Nanda Kishore Panigrahi, cardiologist at Apollo Hospitals, Visakhapatnam, delivered a presentation as an invited faculty and master presenter at Trans Catheter Therapy(TCT) conference held at San Francisco, from October 25 to 28.
A challenging case performed at Apollo Hospital, Visakhapatnam, was selected for presentation. The title of the case was “Retro wiring and antegrade valve delivery during Trans catheter mitral valve replacement (TMVR) in the presence of septal aneurysm and after failed antegrade delivery.”
The technique was applied in a patient who had previously undergone valve replacement surgery and was suffering from recurrent heart failure with repeated hospitalisation. This innovative manoeuvre assumes significance when septal puncture is imprecise and difficult in the presence of septal aneurysm and the catheter-based mitral valve cannot percutaneously deliver antegrade, shared Dr. Panigrahi.
“The case also shows how downward traction by a snare at the abdominal aortic end of the guide wire, after retrograde wiring and artery veino-arterial loop, provided additional support to negotiate the valve without surgery,” he added.
